Powerhouse Wrestling Club Waiver and Release from Liability

1. I. _________________________, the undersigned, on behalf of myself, my heirs and next of kin, personal representatives, agents, insurers, successors and assigns (all hereinafter “Releasors”) hereby FOREVER RELEASE, DISCHARGE AND COVENANT NOT TO SUE Council Bluffs Panthers Wrestling Club, dba Powerhouse Wrestling Club, its affiliated clubs, administrators, agents, directors, officers, members, committees, volunteers, all employees of Powerhouse Wrestling Club, and any and all participants, officials, referees, coaches, host clubs, sponsoring agencies, sponsors, advertisers, local organizing committees (and if applicable, owners, lessors and operators of premises used to conduct any Powerhouse Wrestling Club sanctioned event, meet, practice, camp or activity (all hereinafter “Releasors”) from any and all liabilities, claims, demands, causes of action or losses of any kind or nature, past, present, or future, direct or consequential that I may hereafter have for PERSONAL INJURY, PERMANENT, TEMPORARY, TOTAL OR PARTIAL DISABILITY, DISFIGUREMENT, PARALYSIS AND ANY OTHER LOSSES OR DAMAGES TO PERSON OR PROPERTY OR DEATH, arising out of participation in, attendance at or traveling to and from any Powerhouse Wrestling Club sanctioned event or activity including, but not limited LOSSES CAUSED BY [PASSIVE OR ACTIVE NEGLIGENCE OF THE RELEASES, or hidden, latent or obvious defects in the facilities or equipment used.

2. Releasor understands and acknowledges that Powerhouse Wrestling Club activities and the sport of wrestling in general have inherent dangers that no amount of care, caution, training, instruction, supervision or expertise can eliminate. RELEASOR EXPRESSLY AND VOLUNTARILY ASSUMES ALL RISK OF PERSONAL INJURY, PERMANENTLY, TEMPORARY, TOTAL OR PARTIAL DISABILITY, DISFIGUREMENT, PARALYSIS AND ANY OTHER LOSSES OR DAMAGES TO PERSON OR PROPERTY OF PASSIVE OR ACTIVE NEGLIGENCE OF THE RELEASEES, or hidden, latent or obvious defects in the facilities or equipment used.

3. Releasor acknowledges and fully understands that each participant in any Powerhouse Wrestling Club sanctioned event, meet, practice, camp or activity, including Releasor, will be engaging in activities that involve risk of serious injury, including permanent, temporary, total or partial disability, disfigurement, paralysis and any other losses to person or property including death, and that severe social and economic losses may result not only from Releasor's own actions or negligence, but from the actions, inactions or negligence of others notwithstanding the rules of play or the condition of premises or of any equipment used. Further Releasor acknowledges and fully understands that there may be other associated risks with such activities which are not known or not reasonably foreseeable at this time. I ACKNOWLEDGE THAT I HAVE SUFFICIENT OPPORTUNITY TO REVIEW THE PROVISIONS OF THIS DOCUMENT AND UNDERSTANDS ITS PURPOSE, MEANING AND INTENT.

PARENTS CODE OF CONDUCT:

·         I will remember that children participate to have fun and that the sport is for youth not adults.

·         I will inform the coaches of any physical disability or ailment that may affect the safety of my child or the safety of others.

·         I will a be positive role model for my child and encourage sportsmanship by showing respect and courtesy and by demonstrating positive support for all wrestlers, coaches, officials and spectators at every practice, camp, tournaments and other events.

·         I will not engage in any kind of unsportsmanlike conduct with any officials, coaches, athletes or parents such as booing and taunting, refusing to shake hands or using profane language or gestures.

·         I will not encourage any behaviors or practices that would endanger the health and well-being of the athletes.

·         I will teach my child to obey the rules and to resolve conflicts without resorting to hostility or violence.

·         I will demand that my child treat other wrestlers, coaches, officials and spectators with respect regardless or race, creed, color, gender or ability.

·         I will teach my child that doing one’s best is more important than winning, so that my child will never feel defeated by the outcome of a match or his/her performance.

·         I will praise my child for competing fairly and trying hard to make my child feel like a winner every time.

·         I will never ridicule or yell at my child or other participants for making a mistake or losing a competition.

·         I will emphasize skill development and practices and how they benefit my child over winning.

·         I will promote the emotional and physical well-being of the athletes ahead of any personal desire I may have for my child to win.

·         I will respect the officials and their authority during competition and will not confront coaches during a match.

·         I will demand a sports environment for my child that is free from drugs and alcohol and I will refrain from their use at events.

·         I will be sure to have my wrestler(s) sign in at every practice and double check to make sure there are not special notes or business that needs to be taken care of at the front desk.

 

Parents as well as their athletes, will be held responsible for compliance with their Code of Conduct.

POWERHOUSE WRESTLERS CODE OF CONDUCT:

·         I will HAVE FUN!  First and foremost, wrestling should be fun for everyone

·         I will go to the bathroom BEFORE practice, going during practice keeps your partner from getting better.

·         I will respect our practice room.

·         I will not wear my wrestling shoes outside of the wrestling room or competition.

·         I will wrestle to the best of my ability at all practices and tournaments.  (“I CAN’T” is not in our vocabulary)

·         I will be 5 minutes early to a scheduled practice.  This will give me time to get ready so practice can begin on time.

·         I will come to every practice.  If I have to miss practice I will notify the coach.

·         I will not cry on the wrestling mat when I lose.  Everyone wants to win but there is always someone that loses.  After a match h I will:

o   Congratulate my opponent and the opposing coaches

o   Leave the mat with my chin up

o   Find the coach when I am ready to discuss the match

·         I will at all times conduct myself in a mature and respectful manner

o   I will not yell or cheer if a wrestler is hurt.

o   Win or Lose, I WILL shake the hands and congratulate my opponent.

o   I will not use vulgar or obscene gestures.

·         I will tell coaches of any injuries.  Wrestling is a combative contact sport; I will get bumps, bruises, scrapes and pulled muscles from time to time.

·         I will be a good sport at ALL TIMES.  It is human nature to want to win but win or lose I will be respectful and gracious.

·         I will treat all my teammates with respect.  (They are my wrestling family and should be treated as such at all times)

·         I will control myself when not practicing or wrestling in a match.  I will support my team when I am not wrestling!

·         I will listen to the coaches.  This means eyes on the coaches with mouths closed.

·         I will sign in at every practice.

·         I will WORK HARD AT ALL TIMES.  Hard work beats talent when talent doesn’t work hard!

Violations of the code of conduct will result in disciplinary actions voted upon by the board members up to and including dismissal from the team.
